Vaccine-derived polio spreads in Africa after defeat of wild virus

Use of the oral polio vaccine was discontinued in the UK in 2004 and the US in 2000, and the UN agency advises that the use of the oral vaccine should be discontinued after polio is judged to be eradicated because of the risk of vaccine-derived outbreaks.

If I understand this article correctly, the vaccine caused outbreak happened in Chad and then spread in the wild to Sudan. It looks like the vaccine whose use caused the Chadian outbreak is the Sabin oral polio vaccine introduced in the US in 1961. If it's the vaccine that was given using sugar cubes in the early-mid 60s that is the polio vaccine I received. The oral polio vaccine is on WHO's List of Essential Medicines (as are chloroquine and hydroxychloroquine).

Based on my understanding of the article I linked, it sounds like the problem was in using the vaccine in an unsuitable environment. What happens, apparently, is that an immunized patient sheds weakened viruses in their feces for a time, other catch it when there is poor sanitation, and over time in circulation the weakened viruses mutate until, some generations of the virus later it is potent enough to cause polio. The quick summary is that the UN effort used the vaccine in an environment where a minor known risk became reality.

The UN has some history of doing mass projects in environments not entirely understood by the UN people carrying it out. A tragic example was the UNICEF effort to dig wells throughout rural Bangladesh to provide safe water. While the wells provided water without all the diseases, UNICEF didn't test their wells for arsenic (hint: if you support a well digging charity, make sure they test the ground water properly and thoroughly). The result is a massive tragedy in rural Bangladesh.
